The Barcode Number and Lot Number can be found on the side of each of your boxes of PureVision Contact Lenses. Refer to the image on the form next to where these details must be inserted to see what these look like. It doesn’t matter which box you take these details from, as long as both the barcode and lot number are off the same box. The site will require that you enter a certain number of digits, so ensure that you enter the numbers exactly as they appear on the box (validation will be performed to ensure number combinations are valid and match purchase receipts and sales data provided by your eye care professional).
You can use the Barcode and Lot Number off any of the boxes you have received as part of your purchase of a 12-months supply, however both numbers must be taken from the same box (i.e. you cannot use the Barcode Number off one box and the Lot number off another). It doesn’t matter which of the boxes you use – all boxes purchased under the one receipt are valid (note that you cannot enter the details of a box received as part of a prior purchase).
Yes, you simply need to make a copy of your receipt before you give the original to your health care fund. Before claiming your redemption or going to your health care provider, make a photocopy of your receipt. You can then attach this copy to the PureVision cashback claim form and send this to us as your proof of purchase (note that validation will be conducted to ensure all receipts are legitimate receipts issued by licensed and registered eye care professionals).
Ask your eye care professional or optometrist for a copy of your original receipt. Note that Bausch & Lomb cannot contact your eye care professional in the event that they are unable to provide you with a replacement receipt. No. A credit card receipt does not provide the level of detail required to verify that the requirements of the offer have been met. Only a formal purchase receipt issued by a participating retailer and demonstrating that you have purchased a full 12-months supply during the promotional period, constitutes a valid receipt
If your claim has been deemed invalid for any reason, you will a receive a letter from Bausch & Lomb no later than 4 weeks after receipt of your details, which will explain why your claim was not deemed valid. In some instances this may be because not all required information was supplied. In others it may be because your claim form was incomplete or contained invalid data, or because there was more than one claim under your name or receipt number, or more than 3 claims were made for your household. If you have any queries relating to why your claim was deemed invalid, please contact the Bausch & Lomb Customer Service team for your country via the details listed above. Please wait until you have received correspondence from Bausch & Lomb before calling, as your claim may not have yet been processed.
Yes, children under 18 are eligible to receive the cashback, as long as they have the consent of a parent or guardian. This consent must be indicated on the online form by ticking the box, or through the signature of the consenting parent, on the form in-store. The $100 money order will be made out to the name entered in the form, so if you do not wish for your child to receive the cashback directly, please enter your own details in this part of the site. Please note however that there is a maximum of ONE claim per person. This means that if you purchase contact lenses for both yourself AND your child, you will be eligible for only one cashback claim, unless you enter your name in your own claim, and your child’s name in the other. There is a maximum of 3 claims per household (where ‘household’ means residence: each claim must be submitted under a unique name).
No, you will be sent a Money Order to the address nominated on your form. This Money Order can be taken to your local post office and changed into cash. There is no cost for this service.
No, you cannot access previously submitted forms in order to edit details or reprint the form. If you have not yet sent in your original or photocopied receipt, you can re-enter your details from the beginning, making sure to print the form this time. Your duplicate details will be removed from the system by our validation team.

Unfortunately your claim will be deemed invalid if you neglected to attach the print-out of your form before sending your receipt, or vice versa. If you have made a back-up copy of either document you can resubmit your details. For this reason we always encourage you to make a photocopy of your receipt and print an additional copy of the online form before you send it to us.
There is no functionality to log in to the site to check the status of your claim, or where it is up to in the validation process, you must simply wait to receive correspondence confirming the success or otherwise of your claim. You will receive this no longer than 4 weeks after you have submitted both your online form AND your printed form and receipt by post. If you have not received a response by then, please wait an additional 2 weeks (i.e. 6 weeks all up) before calling Bausch & Lomb Customer Service for your country.
