HOW TO CLAIM

  1. To be eligible to claim, individuals with a contact lens prescription must purchase, in a single transaction, a 12 month supply of PureVision contact lenses, as outlined below:

    1. 8 boxes of PureVision Sphere or PureVision Toric Contact Lenses (3 lenses in a box); or
    2. 4 boxes of PureVision Multifocal Contact Lenses (6 lenses in a box)

    from a participating retailer in Australia or New Zealand (either in-store or online), between 8/2/10 and close of business on 31/5/10 (for in-store purchases) or 11.59pm AEST (for online purchases).

    Any individuals under 18 years old must have parental/guardian consent to claim.

  2. To claim, individuals must then send in their claim via one of the following ways:

    Australian residents:

    1. complete the claim form found in-store or complete and print out the online claim form at www.purevision.com.au/cashback. The claim form must include a valid product barcode number, valid product lot number (i.e. the 9 digit number located beneath the product barcode number) and full purchase receipt number;
    2. attach the original or a copy of the purchase receipt to the claim form; and
    3. send to “PureVision $100 Cashback Offer, PO Box 346, North Ryde NSW 1670 Australia” so that it is received by last mail received on 14/6/10.

    New Zealand residents:

    1. complete the claim form found in-store or complete and print out the online claim form at www.purevision.com.au/cashback. The claim form must include a valid product barcode number, valid product lot number (i.e. the 9 digit number located beneath the product barcode number) and full purchase receipt number;
    2. attach the original or a copy of the purchase receipt to the claim form; and
    3. send to “PureVision $100 Cashback Offer, PO Box 24, 138 Royal Oak, Auckland, 1003, New Zealand” so that it is received by last mail received on 14/6/10.

TERMS AND CONDITIONS

  1. Instructions on how to claim the reward form part of these Terms and Conditions. Participation in this offer is deemed acceptance of these Terms and Conditions. Offer is not valid in conjunction with any other offer of the Promoters.
  2. Claims are only open to Australian and New Zealand residents who have a contact lens prescription. Claimants under 18 years old must have parental/guardian approval to claim and further, the parent/guardian of the claimant must read and consent to all terms and conditions. Parents/guardians may be required by the Promoters to enter into a further agreement as evidence of consent to this Offer.
  3. Employees (and their immediate families) of the Promoters, participating retailers and agencies associated with this offer are ineligible to claim. Immediate family means any of the following: spouse, ex-spouse, de-facto spouse, child or step-child (whether natural or by adoption), parent, step-parent, grandparent, step-grandparent, uncle, aunt, niece, nephew, brother, sister, step-brother, step-sister or 1st cousin.
  4. The Promoters reserve the right, at any time, to verify the validity of claims and claimants (including a claimant’s identity, age and place of residence) and to disqualify any claimant who submits a claim that is not in accordance with these Terms and Conditions or who tampers with the claim process. Errors and omissions will be accepted at the Promoters’ discretion. Failure by the Promoters to enforce any of their rights at any stage does not constitute a waiver of those rights.
  5. Incomplete, indecipherable or illegible claims will be deemed invalid.
  6. Limit of one claim per person.
  7. Multiple claims per household permitted, subject to the following: (a) only one claim permitted per purchase receipt; (b) each claim must be submitted separately and in accordance with claim requirements; and (c) limit of three claims per household.
  8. Claimants must retain their original or a copy of their purchase receipt(s) and product barcodes for all claims as proof of purchase. Failure to produce the proof of purchase for all claims when requested may, in the absolute discretion of the Promoters, result in invalidation of ALL of a claimant’s claims and forfeiture of any right to a reward. Purchase receipt(s) must clearly specify the retailer of purchase and that the purchase was made during the offer period.
  9. If there is a dispute as to the identity of a claimant, the Promoters reserve the right, at their sole discretion, to determine the identity of the claimant.
  10. Offer commences on 8/2/10 and ends for purchases on 31/5/10 at close of business (in-store purchases) and 11.59pm AEST (for online purchases). Final claims close last mail received on 14/6/10.
  11. Successful claimants will be notified in writing.
  12. The Promoters’ decision is final and no correspondence will be entered into.
  13. Every valid claim received will be eligible to receive AU/NZ$100 cash back (whichever is applicable), awarded in the form of a money order. The money order will be sent to the successful claimant at the address specified on the claim form.
  14. The reward is subject to the standard terms and conditions of individual reward and service providers.
  15. If for any reason a claimant does not take a reward, or an element of the reward, by the time stipulated by the Promoters, then the reward or that element of the reward will be forfeited.
  16. Rewards, or any unused portion of a reward, are not transferable or exchangeable.
  17. If this offer is interfered with in any way or is not capable of being conducted as reasonably anticipated due to any reason beyond the reasonable control of the Promoters, the Promoters reserve the right, at their sole discretion, to the fullest extent permitted by law (a) to disqualify any claimant; or (b) to modify, suspend, terminate or cancel the offer, as appropriate.
  18. Any cost associated with accessing the claim website is the claimant’s responsibility and is dependent on the internet service provider used. The use of any automated software or any other mechanical or electronic means that allows a claimant to automatically claim repeatedly is prohibited and will render all claims submitted by that claimant invalid.
  19. Except for any liability that cannot be excluded by law, the Promoters (including their officers, employees and agents) exclude all liability (including negligence), for any personal injury; or any loss or damage (including loss of opportunity); whether direct, indirect, special or consequential, arising in any way out of the Offer, including, but not limited to, where arising out of the following: (a) any technical difficulties or equipment malfunction (whether or not under the Promoters’ control); (b) any theft, unauthorised access or third party interference; (c) any claim or reward that is late, lost, altered, damaged or misdirected (whether or not after their receipt by the Promoters) due to any reason beyond the reasonable control of the Promoters; (d) any variation in reward value to that stated in these Terms and Conditions; (e) any tax liability incurred by a claimant; or (f) use of a reward.
  20. As a condition of accepting a reward, the successful claimant (or his/her parent or nominated guardian) must sign any legal documentation as and in the form required by the Promoters and/or prize suppliers in their absolute discretion, including but not limited to a legal release and indemnity form.
  21. The Promoters collect personal information in order to conduct the Offer and may, for this purpose, disclose such information to third parties, including but not limited to agents, contractors, service providers, reward suppliers and as required, to Australian and New Zealand regulatory authorities. Claim is conditional on providing this information. The Promoters may, for an indefinite period, unless otherwise advised, use the information for promotional, marketing, publicity, research and profiling purposes, including sending electronic messages or telephoning the claimant. Claimants should direct any request to access, update or correct information to the Promoters. All claims become the property of the Promoters.
  22. The laws of Australia apply to this offer to the exclusion of any other law. Claimants submit to the exclusive jurisdiction of the courts of Australia.
